When I was trying to fill in a form, the check boxes would not show a checkmark - they stayed blank. But if I printed the form, a pair of scissors showed up. Turns out the problem was that I had the ITC Zapf Dingbats font installed and it was being used. But for forms to work correctly, Acrobat wants to use its substitute font, AdobePiStd. When I removed Zapf Dingbats, the form worked correctly.

To see the fonts in use in either Reader or full Acrobat, look at File > Properties > Fonts. It will also show substitutions.
